Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. We do not bill any insurance company but we provide you with a superbill to provide to your insurance company for reimbursement. We have opted out of Medicare. If you have Medicare you will not get reimbursed and cannot bill Medicare or your Secondary.
Payment
Cash, check and all major credit cards accepted for payment at the time of your appointment.
Cancellation Policy
If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you may be subject to cancellation fees.
